2010 Activity Report (2): Peer Visit

(1) Date: February 23, 2010
Venue: Mr. A’s home (1st peer visit)

Objectives of 1st peer visit:
1. To introduce life haven independent living center.
2. To know the general condition of the visited person with disability.
3. To know what help/assistance life haven independent living center can extend to the visited person with disability.
4. To encourage the visited person with disability to join the activities of life haven independent living center.
Participants: Abner Manlapaz, Teody Auxilian & Eric Dy

Outcome: Mr. A warmly welcomed the LHILC staff. All the objectives were met. The center provide Mr. A a wheelchair for his mobility. Now, he can move around with ease.

(2) Date: February 23, 2010
Venue: Mr. B’s home (1st peer visit)

Participants: Abner Manlapaz & Eric Dy

Outcome: Mr. B welcomed Abner and Eric. All the objectives were met.

(3) Date: March 10, 2010
Venue: Mr. C’s home (1st peer visit)

Participants: Abner manlapaz & Meanne Dao

Outcome: Abner and Meanne shared the IL Philosophy. The LHILC and its services were introduced to Mr. C. The needs of Mr. C for personal assistance and wheelchair were identified. The center provided Mr. C wheelchair for his mobility. Now, he can go out of their home and join the activities in their neighborhood.

(4) Date: March 29, 2010
Venue: Mr. B’s home (2nd peer visit)
Objectives of 2nd visit:
1.To follow-up on Mr. B.
2.To encourage Mr. B some more to go out and join the coming support group activity.

Participants: Jun Bernandino & Eric Dy

Outcome: Mr. B, on this second visit, has more time for the sharing. He agreed to join the support group activities being organized by the center.

(5) Date: April 8, 2010
Venue: Ms. D’s home (1st peer visit)

Participants: Eric Dy & Teody Auxilian (1st)

Outcome:
1.Life haven independent living (LHILC) staff successfully introduced LHILC and its services to Ms. D and her family.
2.LHILC staffs learned the general condition of Ms.D and her family and her need for a powered wheelchair.

(6) Date: April 13, 2010
Venue: Ms. E’s home (1st peer visit)

Participants: Jun Bernandino & Meanne Dao

Outcome: All the objectives were met. The center also provided Ms. E walker to help her on her mobility.

(7) Date: April 17, 2010
Venue: Ms. F’s home (1st peer visit)

Participants: Eric Dy & Teody Auxilian

Outcome:
1.LHILC staffs successfully introduced the center and its services to Ms. F’s family.
2.LHILC staffs learned the general condition of Ms. F and her family and her need for a wheelchair.
3.The center provide Ms. F a new wheelchair. Now, her parents no longer need to carry Ms. F whenever they need to move her from one place to another.

(8) Date: July 16, 2010
Venue: Mr. G’s home (1st peer visit)

Participants: Jun Bernandino & Eric Dy

Outcome:
1.LHILC staffs successfully introduced the center and its services to Mr. G and his family.
2.LHILC staffs learned the general condition of Mr. G and his family and his need for a wheelchair.

(9) Date: August 21, 2010
Venue: Ms. D’s home (2nd peer visit)

Participants: Jun Bernandino & Eric Dy (2nd)

Outcome: LHILC staffs follow up on her about her situation. Ms. D open up again that she need a powered wheelchair for her plans to become more productive to help in the household income. The staffs explained to Ms. D about the responsibilities and how to maintain a powered wheelchair.

(10) Date: August 21, 2010
Venue: Ms. H’s home (1st peer visit)

Participants: Jun Bernandino & Eric Dy

Outcome:
1. LHILC staffs introduced the center and its services to Ms. H’s family.
2. LHILC staffs learned the general condition of Ms. H and her family and her need for a wheelchair. Ms. H has intellectual and mobility disability.

(11) Date: August 26, 2010
Venue: Mr. I’s home (1st peer visit)

Participants: Eric Dy & Teody Auxilian

Outcome:
1. LHILC staffs successfully introduced the center and its services to Ms. H’s family.
2. LHILC staffs learned the general condition of Ms. H and his family and his need for a wheelchair.
3. LHILC staffs shared information to Ms. H’s parents about his condition and made some referrals.
4. The parents expressed willingness to participate in LHILC activities.

(12) Date: September 1, 2010
Venue: Life Haven Independent Living Center (Mr. J decided to visit the office instead of us visiting him.) (1st visit)

Participants: LHILC staffs w/ Arnel Aromyn

Outcome:
1. LHILC staffs successfully introduced the center and its services to Mr. J.
2. LHILC staffs learned the general condition of Mr. J and his need for a wheelchair.
3. It is very apparent that Mr. J is interested to join in all the activities of LHILC.

(13) Date: September 1, 2010
Venue: Ms. K’s home (1st peer visit)

Participants: Jun Bernandino, Eric Dy, Cris Estigoy & Manny Payumo

Outcome:
1. LHILC staffs successfully introduced the center and its services to Ms. K.
2. LHILC staffs learned the general condition of Ms. K.
3. It is very evident that Ms. K is interested to join in all the activities of LHILC.

(14) Date: September 1, 2010
Venue: Mr. L’s home (1st peer visit)

Participants: Jun Bernandino, Eric Dy & Cris Estigoy

Outcome: LHILC staffs successfully introduce the center. He is also willing to attend the activities of the center depending on his availability. He is still undergoing therapy. Also, his mother is not supporting the idea of his son joining the activities of the center.

(15) Date: September 4, 2010
Venue: Mr.C’s home (3rd peer visit)
Objective of 3rd peer visit:
1. To follow-up on the condition of Mr. C.
2. To know why he cannot attend the support group activities.

Participants: Teody Auxilian & Meanne Dao

Outcome: Mr. C is willing to attend but cannot because he cannot get assistance if his mother is not at home. Mr. C also have difficulty waking up early because he is used to wake up late. The reason for this is because his mother is usually working from the morning until 12 noon. Because of this, no one is going to help Mr. C if he wakes up early than the usual.

(16) Date: September 4, 2010
Venue: Ms. M’s home (2nd peer visit)

Participants: Teody Auxilian & Meanne Dao

Outcome: The staffs found out that Ms. M cannot go out of their house because it is not accessible inside and outside their house. In order for her to go out, she needs to be carried out. She also mentioned about her interest to get a prosthesis. But she may not be able to get it because she plan to return to their province.

(17) Date: September 10, 2010
Venue: Mr. N’s home (1st peer visit)

Outcome:
1. LHILC staffs successfully introduced the center and its services to Mr. N.
2. LHILC staffs learned the general condition of Mr. N and his need for a wheelchair.

(18) Date: September 10, 2010
Venue: Mr. O’s home (1st peer visit)

Participants: Eric Dy & Teody Auxilian

Outcome: Mr. O welcomed the LHILC staffs. He was gladdened to see the staffs. LHILC staffs introduce IL.

(19) Date: September 10, 2010
Venue: Mr. P’s home (1st peer visit)

Participants: Eric Dy & Teody Auxilian

Outcome: Mr. P welcomed the staffs from the center. The center introduced independent living and the services of the center. Mr. P was amazed that persons with extensive disabilities visit him in his house.
